Most people treat dark circles as one condition. Clinically, they are at least three, and the treatments for each barely overlap. A brightening cream built for pigment will do almost nothing for circles caused by visible blood vessels. A filler designed to correct a hollow will not touch melanin. This is why so many under-eye products disappoint: the buyer never identified which problem they were solving.

Dermatologists use a quick physical assessment, often called the stretch test or distraction test, as a first-pass sorting tool. You can do a version of it at home in under a minute with a mirror and decent daylight.

How to do it. Stand facing a window with indirect natural light. Look straight ahead, then gently pull the skin of the lower eyelid downward and slightly outward with one clean finger, stretching it taut against the bone of the cheek. Watch what happens to the dark color.

If the darkness gets worse or spreads when stretched, the likely driver is vascular. The thin skin of the lower eyelid, often less than one millimeter thick, becomes even thinner when stretched, so the network of small veins and capillaries beneath it shows through more clearly. Vascular circles tend to look blue, purple, or maroon, worsen with poor sleep, allergies, or nasal congestion, and often run in families with fair or thin skin. Mechanistically, the color comes from deoxygenated blood pooling in vessels close to the surface, plus light scattering through translucent skin.

If the darkness stays exactly the same when stretched, and the color reads brown or gray-brown rather than blue, the driver is likely pigmentary. This is excess melanin sitting in the epidermis or upper dermis. It is more common in medium to deep skin tones, in people with atopic dermatitis or chronic eye rubbing, and after inflammation from allergies or harsh skincare. Pigment does not move when you stretch the skin because it is embedded in the tissue itself. A second confirmation: pigmentary circles often extend slightly onto the upper eyelid or the sides of the nose, following the pattern of periorbital melanosis.

If the darkness improves or disappears when stretched, or when you tilt your chin up and shine light directly at your face, you are probably looking at a structural shadow. This is not discoloration at all. It is a groove, usually the tear trough, where the lower eyelid meets the cheek. Volume loss in the fat pads under the eye, a naturally deep orbital rim, or descent of the midface with age creates a valley. Overhead light cannot reach into the valley, so it reads as a dark band. Stretching the skin flattens the groove and the shadow vanishes. A photo taken with flash pointed straight at the face, which fills the shadow, versus one taken under overhead lighting, which deepens it, will show a dramatic difference.

Why the distinction matters. The treatment map splits cleanly along these lines. Pigmentary circles respond to ingredients and procedures that reduce or disperse melanin: topical retinoids, azelaic acid, vitamin C, kojic acid, careful use of hydroquinone under medical supervision, strict daily sunscreen, and in-office options such as low-strength chemical peels or pigment-targeting lasers. Vascular circles respond to a different toolkit: managing allergies and nasal congestion, cold compresses, caffeine-containing topicals that temporarily constrict vessels, retinoids to gradually thicken the dermis, and vascular lasers in select cases. Structural circles are a contour problem, so the interventions are volumetric: hyaluronic acid filler placed by an experienced injector, fat repositioning during lower blepharoplasty, or simply strategic lighting and makeup that fills shadow optically rather than chemically.

The complication: most people are mixed. Studies examining periorbital hyperpigmentation consistently find that a majority of patients have two or more contributing mechanisms at once. A common combination is mild tear trough hollowing plus vascular show-through, which stretching partially improves and partially worsens, creating a confusing result. If your stretch test is ambiguous, the honest interpretation is that you have a blended presentation and may need a layered approach.

What the test cannot tell you. It will not distinguish epidermal from dermal pigment, which affects how well topicals penetrate. It will not detect contributing medical issues such as thyroid dysfunction, anemia, or chronic allergic rhinitis, all of which can worsen periorbital darkness and deserve evaluation if circles appeared suddenly or worsened rapidly. And it cannot replace a Wood's lamp examination or an in-person assessment when you are considering procedures.

Still, as a free screening tool, the stretch test earns its place. Thirty seconds in front of a mirror can save months of spending on products designed for a problem you do not have. Identify the mechanism first. Then, and only then, choose the treatment.
