Most under-eye treatments fail because people treat the wrong type of dark circle. A simple at-home assessment, borrowed from dermatology exam rooms, can tell you which of three mechanisms you are actually dealing with.

Walk into any pharmacy and you will find a wall of products promising to erase dark circles. What the packaging rarely mentions is that dermatologists do not treat dark circles as a single condition. In clinical literature, periorbital hyperpigmentation is sorted into at least three distinct mechanisms, and each responds to completely different interventions. Treating one type with a remedy designed for another is the most common reason people conclude that nothing works.

The good news is that a rough version of the assessment clinicians use takes about two minutes, a mirror, and decent lighting. Here is how to run it, what each result means, and why the mechanism matters more than the marketing.

Step one: the stretch test. Stand near a window with indirect daylight. Gently pull the under-eye skin downward and slightly outward with one clean finger. Watch what happens to the color. If the darkness stays exactly the same or gets slightly more visible as the skin stretches, you are most likely looking at true pigment, meaning excess melanin deposited in the epidermis or dermis. If the darkness fades or disappears when the skin is taut, the color is probably coming from structure or vasculature, not from pigment in the skin itself.

Step two: the light test. Now shine light directly at your face, then compare with light coming from above, the way overhead office lighting hits you. If the circles nearly vanish under direct frontal light but look dramatic under overhead light, the primary problem is shadow. This is caused by anatomy: a groove called the tear trough where the orbital rim meets the cheek, volume loss under the eye, or mild puffiness above the groove that deepens the valley below it. Shadow circles are an optical phenomenon. There is no discoloration in the skin at all, which is why no cream, no matter how expensive, can meaningfully change them.

Step three: the color check. If the stretch test suggested the color is not pigment, look closely at the hue. A bluish, purplish, or reddish cast points to a vascular cause. The skin under the eye is among the thinnest on the body, often under half a millimeter, and it sits directly over a dense network of small veins and the orbicularis muscle. When that skin is thin, the blood beneath shows through, the same way veins are visible on the inner wrist. Vascular circles typically worsen with poor sleep, allergies, nasal congestion, and alcohol, because all of these increase blood pooling or vessel dilation in the area. Brownish or grayish tones that persist regardless of sleep point back to melanin.

Why the distinction changes everything. Consider the standard toolkit. Brightening ingredients such as azelaic acid, vitamin C, kojic acid, and low-strength retinoids work by interrupting melanin production or speeding cell turnover. They can genuinely help pigment-type circles over 8 to 12 weeks, but they do nothing for a shadow cast by a tear trough and little for blood visible through thin skin. Caffeine-based eye products temporarily constrict superficial vessels, which can modestly soften vascular circles for a few hours, but caffeine has no effect on melanin and none on anatomy. In-office options follow the same logic: pigment responds to certain lasers and chemical peels, vascular show-through is sometimes addressed by thickening the skin with retinoids or collagen-stimulating procedures, and shadow is fundamentally a contour problem that only volume-based or structural approaches can address.

The complication: most people are mixed. Studies of periorbital hyperpigmentation consistently find that a majority of patients have more than one mechanism at once, most commonly pigment layered over a vascular component, or vascular circles deepened by an early tear trough. This is why partial improvement is a realistic outcome and total erasure often is not. It also explains a familiar frustration: a brightening serum improves things noticeably, then progress stalls, because the remaining darkness was never pigment to begin with.

A note on genetics and ethnicity. Pigment-dominant circles run strongly in families and are more prevalent in people with medium to deep skin tones, where periorbital melanocytes are naturally more active. In these cases, sun protection is not optional. Ultraviolet exposure directly stimulates the melanin that causes the darkness, and any brightening regimen without daily sunscreen around the eyes is working against itself.

The bottom line. Before spending money on the next promising product, run the stretch test and the light test. If the color survives stretching, think pigment and look at evidence-backed brighteners plus sun protection. If it fades and looks bluish, think vascular, and prioritize sleep, allergy control, and skin-thickening strategies. If it disappears under frontal light, you have a shadow, and no topical will fill a valley. Two minutes at the mirror will save you months of treating the wrong problem.
