The Stretch Test: How Clinicians Tell Pigment, Vessels, and Shadow Apart Before Treating Dark Circles
Most under-eye treatments fail because they target the wrong cause. A simple two-minute exam, borrowed from dermatology clinics, can tell you whether your circles are pigment, blood vessels, or pure anatomy.

Under-eye dark circles are usually discussed as one problem, but dermatologists treat them as at least three distinct conditions that happen to look similar in a mirror. Getting the diagnosis wrong is the single most common reason a product or procedure disappoints. A brightening cream will do nothing for a circle caused by a hollow, and filler will do nothing for one caused by melanin. Before spending money on either, it helps to run the same low-tech assessment clinicians use in the exam room.
The three mechanisms behind the shadow. The first is pigmentary: excess melanin deposited in the skin of the lower eyelid. This is often genetic, more common in deeper skin tones, and can be worsened by rubbing, allergies, sun exposure, and postinflammatory changes after eczema flares. The second is vascular: the eyelid skin here is among the thinnest on the body, often under half a millimeter, so the bluish or purplish network of veins and capillaries beneath it shows through. Fatigue, dehydration, allergies, and anything that dilates or congests those vessels makes this type look worse. The third is structural: there is no discoloration at all, only a groove called the tear trough where the orbital rim meets the cheek. Light hitting the face casts a shadow into that valley, and the shadow reads as darkness. Volume loss with age, a naturally deep-set orbit, or descent of the cheek fat pad all deepen it.
Test one: the stretch. Look in a mirror under even, indirect light. Gently pull the skin under one eye downward and slightly outward with a clean finger. If the darkness stays exactly as dark while the skin is stretched, pigment is the likely driver, because melanin sits in the skin itself and moves with it. If the darkness gets worse or spreads as the skin thins under tension, think vascular, since stretching makes thin skin even more transparent over the vessels below. If the darkness largely disappears when the skin is pulled taut, you have flattened the groove and eliminated the shadow, which points to a structural cause.
Test two: change the light. Take two photos of your face, one with light coming from directly in front, such as facing a window, and one with overhead light, such as a ceiling fixture. Structural circles change dramatically between the two, deepening under overhead light and softening under frontal light. Pigmentary circles look nearly identical in both photos because melanin does not care about the angle of illumination.
Test three: the tilt. Tip your head back and look at the under-eye area in a mirror held above you. If the darkness fades noticeably, shadow from a tear trough is again the likely culprit. Some clinicians also press gently on the area for a few seconds: vascular darkness may blanch briefly and refill, while pigment does not change with pressure.
Why the answer changes the treatment. For pigmentary circles, the evidence supports patience and consistency: daily broad-spectrum sunscreen, topical agents that interrupt melanin production such as azelaic acid, vitamin C, kojic acid, or prescription options a dermatologist may recommend, and strict avoidance of eye rubbing, which drives postinflammatory pigmentation. Chemical peels and certain pigment-targeting lasers are in-office options, but they carry real risk of worsening pigmentation in darker skin if performed aggressively. For vascular circles, the goal is either to constrict or camouflage: caffeine-containing eye products offer a modest, temporary vasoconstrictive effect, managing allergies reduces vessel congestion, and retinoids used carefully over months can gradually thicken the dermis so vessels show through less. For structural circles, no topical will fill a valley. Options include hyaluronic acid filler placed by an experienced injector, or simply strategic light-reflecting concealer, which works precisely because it fights shadow with brightness.
The complication: most people are mixed. Studies examining dark circle etiology consistently find that a large share of patients have two or even all three mechanisms at once, for example mild pigment layered over a shallow tear trough. This is why partial improvement after a single treatment is common and expected, not a sign the treatment failed. It also explains why before-and-after photos from other people are poor predictors of your own result.
When to see a professional. If circles appeared suddenly, affect only one eye, or come with swelling, itching, or pain, get evaluated, since thyroid conditions, allergic dermatitis, and other medical issues can present this way. Otherwise, the stretch test costs nothing, takes two minutes, and will make every subsequent decision about creams, lasers, or filler considerably smarter.
