Ask a dermatologist why so many people spend years on eye creams with nothing to show for it, and you will hear a version of the same answer: the product was never matched to the problem. Under-eye darkness is not one condition. It is at least three distinct conditions that happen to look similar in a bathroom mirror, and each responds to a different set of interventions.

The good news is that clinicians use a simple bedside method to sort them apart, and most of it can be replicated at home with a mirror, a window, and about two minutes. The core maneuver is often called the stretch test, and it works because the three main causes of dark circles behave differently when the skin is pulled taut.

How to do it. Stand facing indirect daylight from a window, not overhead bathroom lighting, which exaggerates shadows. Look straight ahead into a mirror. Place one clean fingertip just below the dark area and gently pull the skin downward and slightly outward, enough to flatten the under-eye surface without discomfort. Watch what happens to the darkness.

If the darkness stays exactly the same, you are most likely looking at true pigmentation, what clinicians call periorbital hyperpigmentation. The color lives in the skin itself as excess melanin, so stretching the skin cannot change it. This type often looks brown or tan rather than blue, frequently extends onto the upper eyelid, and is more common in people with medium to deep skin tones. It can be genetic, but it is also driven by sun exposure, chronic rubbing from allergies or eczema, and post-inflammatory pigment left behind by irritation. The mechanism matters: melanin responds to sun protection, topical agents that slow pigment production such as azelaic acid, vitamin C, or prescription options a dermatologist may recommend, and above all to stopping the rubbing. No amount of caffeine cream or filler addresses melanin.

If the darkness stretches with the skin and may even look worse, the likely culprit is vascular. The skin of the lower eyelid is the thinnest on the body, often under half a millimeter, and in some people it is thin or translucent enough that the network of veins and the underlying orbicularis muscle show through as a blue, purple, or reddish tint. Stretching thins the skin further, so the color intensifies. A second confirmation: this type often looks worse after poor sleep, alcohol, salty food, or during allergy season, because anything that dilates vessels or promotes fluid pooling makes the show-through more visible. Mechanistically, the fixes are about either constricting what shows through, which is why caffeine creams give a modest and temporary effect, or thickening what covers it, which is why retinoids that build collagen over months can produce real but slow improvement.

If the darkness disappears when you stretch, you probably do not have discoloration at all. You have a shadow. This is the structural type, caused by the anatomy of the tear trough, the natural groove where the lower eyelid meets the cheek. When that groove is deep, either from genetics or from age-related loss of fat and bone volume in the midface, overhead light casts a shadow into it. Pulling the skin flattens the groove and the shadow vanishes. You can double-check by tilting your chin up toward a ceiling light and watching the circle fade, or by taking a photo with flash aimed straight at the face, which fills the shadow. Structural circles do not respond to any topical product, because there is no pigment or vessel to treat. They respond to volume-based or light-redirecting approaches, which is why this is the one category where injectable filler placed by a qualified physician has a genuine mechanistic rationale, along with the humbler option of a peach or salmon toned corrector that optically counters shadow.

The complication: most people are mixed. Studies examining periorbital darkening consistently find that a majority of patients have two or more contributing factors at once, most often thin vascular skin sitting inside a mild tear trough. If your stretch test gives a partial result, the darkness fades somewhat but does not disappear, that mixed picture is probably you. In that case the practical order of operations is to address the reversible contributors first: daily sunscreen to the under-eye area, allergy control if you rub your eyes, consistent sleep, and a gentle retinoid if your skin tolerates it, before considering procedures.

One caution worth stating plainly. Darkness that appears suddenly, affects only one eye, or comes with swelling, pain, or vision changes is outside the scope of cosmetic self-assessment and warrants a medical visit. And any injectable treatment near the eye carries real risks and belongs exclusively in the hands of an experienced physician.

The stretch test will not replace a clinical exam, but it will do something most marketing never does: tell you what you are actually treating before you spend money treating it.
