Pigment, blood vessels, and shadow all look similar in the mirror but respond to completely different treatments. A simple at-home check used by dermatologists can point you in the right direction before you spend anything.

Most people who buy an eye cream for dark circles never ask the question that determines whether it can possibly work: what is actually causing the darkness? Under-eye discoloration is not one condition. It is at least three, and each has a different mechanism, a different appearance under scrutiny, and a different set of treatments with evidence behind them. Dermatologists routinely sort patients using a quick physical maneuver often called the stretch test, and it is something you can do at home in under two minutes.

The three main mechanisms. The first is true pigmentation, meaning excess melanin deposited in the skin of the lower eyelid. This is common in people with medium to deep skin tones, tends to run in families, and can be worsened by sun exposure, chronic rubbing from allergies, and eczema. The second is vascular show-through. The skin under the eye is among the thinnest on the body, often under one millimeter, and when it is thin enough, the bluish or purplish network of veins and the muscle beneath become visible. Fluid retention and dilated vessels from poor sleep, alcohol, or allergies can intensify this. The third is structural shadow. As the cheek loses volume with age or genetics creates a deep tear trough, a groove forms between the lower lid and the cheek. Overhead light falls into that groove and casts a shadow that reads as darkness even though the skin itself may be perfectly normal in color.

How to do the stretch test. Stand in front of a mirror in even, indirect light. Place one clean fingertip just below the dark area and gently pull the skin downward and slightly outward, toward your cheekbone. Do not tug hard. You only need enough tension to flatten the skin. Then observe what happens to the color.

If the darkness stays exactly the same, or looks essentially unchanged when the skin is flattened, pigment is the likely driver. Melanin sits within the skin itself, so moving or stretching the skin moves the color along with it. If the darkness gets worse or more intensely blue or purple when stretched, the cause is probably vascular. Stretching thins the skin further, which makes the vessels and muscle underneath show through even more clearly. If the darkness improves or disappears when you stretch, structure and shadow are the likely culprit. Flattening the tear trough groove removes the depression that was catching the shadow, and the color evens out.

A second check with light. You can confirm the shadow finding with a phone flashlight. Shine the light directly at your face from the front, at eye level. Shadow-based circles often vanish under direct frontal light because there is no overhead angle to create a shadow. Pigment and vascular circles remain visible no matter where the light comes from. Photographs taken under bathroom downlights exaggerate structural circles for the same reason, which is why many people look worse in mirrors lit from above than they do outdoors.

Why the distinction matters. Treatments only work when they match the mechanism. Pigment responds to daily sunscreen, topical agents that interrupt melanin production such as azelaic acid, vitamin C, or prescription options a dermatologist may recommend, and in some cases chemical peels or pigment-targeting lasers. None of those will do anything for a shadow. Vascular circles respond modestly to measures that constrict vessels or thicken skin over time, including caffeine-containing topicals for short-term appearance, retinoids for gradual collagen support, and managing allergies and sleep. Structural shadows generally do not respond to any cream, because the problem is a three-dimensional groove, not a color in the skin. They are typically addressed with volume-based approaches evaluated by a qualified physician, or simply accepted and softened with light-reflecting concealer.

The honest caveat: most people are mixed. Real-world dark circles rarely come from a single mechanism. A very common combination is thin skin with visible vessels sitting inside a mild tear trough, with a layer of rubbing-induced pigment on top from years of allergies. If your stretch test gives an ambiguous result, partial improvement with some residual color, that mixed picture is probably why. It also explains why a friend's miracle product did nothing for you. You may simply have a different dominant mechanism.

When to get evaluated. See a board-certified dermatologist if the darkness appeared suddenly, affects only one side, is accompanied by swelling, itching, or scaling, or if you have tried mechanism-matched measures for three to six months without change. One-sided or rapidly changing discoloration deserves a professional look because it can occasionally signal something other than ordinary dark circles.

The stretch test will not replace a clinical exam, but it does something valuable: it turns a vague complaint into a specific hypothesis. Knowing whether you are fighting melanin, blood vessels, or geometry is the single most useful piece of information you can have before spending money on any dark circle treatment.
