Most treatment failures start with a misdiagnosis. A two-minute self-exam borrowed from dermatology clinics can tell you which of the three main types of dark circles you actually have, and why that changes everything about what will work.

Ask a dermatologist why so many people spend years on eye creams with nothing to show for it, and you will hear a version of the same answer: they were treating the wrong problem. Under-eye dark circles are not one condition. Clinicians generally sort them into three categories, each with a distinct mechanism, and each responding to a completely different set of interventions. The good news is that a simple bathroom-mirror exam, sometimes called the stretch test or pinch test, can point you toward the right category before you spend another dollar.

Why the distinction matters

The skin below the eye is the thinnest on the body, often measuring around half a millimeter. That thinness means three separate things can create the appearance of darkness. First, excess melanin in the skin itself, a condition dermatologists call periorbital hyperpigmentation. Second, visible vasculature, where the blue and purple tones of veins and pooled blood show through translucent skin. Third, structural shadow, where a groove called the tear trough, or hollowing from fat and bone changes, creates a literal shadow that reads as darkness even though the skin color is normal. A brightening serum cannot fix a shadow. A filler cannot fix melanin. Matching mechanism to treatment is the entire game.

How to run the test

Stand in front of a mirror with strong, direct light, ideally daylight from a window rather than overhead bathroom lighting, which exaggerates shadows. Then work through three checks.

First, the stretch. Gently pull the under-eye skin downward and slightly outward with one finger. If the darkness stretches and stays visibly brown or tan within the skin itself, pigment is likely a major contributor. Melanin sits in the skin, so it moves when the skin moves.

Second, the color check. Look at the tone of the darkness without stretching. Brown or tan suggests pigment. Blue, purple, or reddish tones suggest a vascular component, meaning the color you see is blood and vessels beneath thin skin. This type often looks worse in the morning, after poor sleep, or during allergy season, because fluid retention and vessel dilation make the vasculature more prominent.

Third, the light test. Tilt your chin up so light hits the under-eye area directly, or shine a phone flashlight straight at the area. If the darkness largely disappears when light fills the hollow, you are looking at structural shadow from a tear trough or volume loss. Shadow-type circles also tend to vanish in selfies taken with front-facing flash and reappear in overhead lighting.

Many people, it should be said, have a mixed picture. Pigment plus shadow is common, especially in people with medium to deep skin tones, and thin skin often coexists with early hollowing as collagen declines with age.

What each result points toward

If the test suggests pigment, the relevant interventions are the ones that address melanin production and turnover: daily broad-spectrum sunscreen, which is arguably the single most evidence-supported step, along with ingredients such as azelaic acid, vitamin C, kojic acid, and retinoids used cautiously on delicate skin. Rubbing the eyes, a habit strongly linked to allergies and eczema, drives pigment through chronic friction and inflammation, so managing the itch matters as much as any serum.

If the test suggests a vascular cause, the logic shifts to reducing vessel prominence and fluid pooling. Sleeping with the head slightly elevated, treating allergies, limiting alcohol and high sodium intake in the evening, and using topical caffeine, which temporarily constricts superficial vessels, can all soften the appearance. Retinoids may help over months by thickening the skin slightly, making the vessels less visible. Cold compresses work for the same constriction reason, though the effect is short lived.

If the test suggests shadow, no topical product will meaningfully help, because the problem is geometry, not color. Options here range from optical camouflage, such as light-reflecting concealers, to procedural approaches like hyaluronic acid filler placed in the tear trough by a qualified injector, or in some cases surgical repositioning of under-eye fat. These carry real risks and costs, which is exactly why confirming that shadow is truly the driver matters before considering them.

The honest caveat

A self-exam is a screening tool, not a diagnosis. Conditions like thyroid disease, anemia, and chronic dermatitis can contribute to under-eye changes, and a board-certified dermatologist can use tools like a Wood's lamp to confirm where pigment sits in the skin. But walking into that appointment already knowing whether your circles stretch, glow blue, or vanish under a flashlight makes the conversation faster and the treatment plan far more likely to work the first time.
