The Stretch Test: How Clinicians Tell What Kind of Dark Circles You Actually Have
Before any cream, laser, or filler makes sense, you need to know whether your dark circles are pigment, blood vessels, or shadow. A simple two-minute self-exam borrowed from dermatology clinics can sort most cases.

Ask three people with dark circles what causes them and you will likely get three different answers: not enough sleep, too much screen time, genetics. All of those can play a role, but they are not the clinical categories that determine treatment. Dermatologists generally sort periorbital darkening into three buckets: pigmented, vascular, and structural. Most people have a mix, but one type usually dominates, and the dominant type dictates what will actually help. The good news is that the same bedside tests clinicians use can be approximated at home with a mirror, a window, and your own fingertip.
Step one: the stretch test. Stand near natural daylight, look straight ahead, and gently pull the skin under one eye downward and slightly outward with a clean finger. Watch what happens to the color. If the darkness stretches with the skin and stays roughly the same shade, the pigment is likely sitting in the skin itself. This points to pigmented dark circles, caused by excess melanin in the epidermis or dermis. Common drivers include genetics, chronic rubbing from allergies or eczema, sun exposure, and post-inflammatory hyperpigmentation, which is more prevalent in medium to deep skin tones. Mechanistically, melanocytes in the thin under-eye skin become overactive or deposit pigment deeper than usual, which is why brightening ingredients that interrupt melanin production, such as azelaic acid, vitamin C, kojic acid, or prescription retinoids, are the logical first-line approach, along with strict sun protection and treating the underlying itch if allergies are involved.
Step two: read the color when the skin is stretched. If stretching makes the darkness fade or the area looks bluish, purplish, or reddish rather than brown, you are probably looking at vascular dark circles. Under-eye skin can be under half a millimeter thick, among the thinnest on the body. That means the orbicularis oculi muscle and the venous plexus beneath it show through, especially in fair skin or when the skin thins with age. Fluid retention, nasal congestion, fatigue, and dehydration can all dilate or engorge these vessels and make the tint more obvious, which is why vascular circles often look worse in the morning or during allergy season. Treatments target either the vessels or the see-through skin: caffeine constricts superficial vessels temporarily, retinoids thicken the dermis over months by stimulating collagen, and certain vascular lasers can reduce visible vessels. Cold compresses work for the same reason they work on a bruise: vasoconstriction.
Step three: the overhead light check. If your circles look dramatic under a bathroom ceiling light but nearly disappear when you face a window or tilt your chin up, the problem is likely structural, meaning shadow rather than color. A groove called the tear trough runs from the inner corner of the eye toward the cheek. When under-eye fat pads bulge above it, or midface volume deflates below it, light hits the ridge and the hollow catches shade. No cream can lighten a shadow. Structural circles respond to volume and contour interventions: hyaluronic acid filler placed carefully in the trough, fat repositioning during lower blepharoplasty, or, in milder cases, simply accepting that the darkness is an optical effect that photographs worse than it looks in person. A practical confirmation: take two selfies, one lit from above and one lit from the front. If the circles vanish with front lighting, structure is your main issue.
Why the distinction matters more than the product aisle suggests. Marketing for eye creams rarely makes these distinctions, and that is where money gets wasted. A brightening serum aimed at melanin will do little for a blue vascular tint. Caffeine will not touch brown epidermal pigment. Filler in a patient whose darkness is purely pigmentary can actually make things look worse by adding a subtle bulge under discolored skin. Clinicians sometimes use a Wood's lamp to confirm pigment depth, since epidermal melanin becomes more pronounced under the lamp while dermal pigment does not, but the stretch test and lighting check capture most of that information for free.
The honest caveats. Mixed presentations are the rule, not the exception. Someone can have dermal pigment, thin skin over visible vessels, and an early tear trough all at once, which is why combination approaches are common and why results from any single intervention are often partial. Persistent or rapidly worsening darkening also deserves a medical look, since thyroid disease, anemia, and chronic allergic disease can contribute. And no test replaces an in-person exam. But two minutes with a mirror and a window will tell you more about your dark circles than most product labels ever will, and it will keep you from paying pigment prices to solve a shadow problem.
