Walk into any pharmacy and you will find a wall of eye creams promising to erase dark circles. What the packaging rarely mentions is that dark circles are not one condition. Dermatology literature generally sorts them into three broad types: pigmented, vascular, and structural, plus mixed cases that combine two or all three. Each has a different mechanism, and a product built for one type often does nothing for another. Before spending money, it helps to know which category you fall into, and there is a simple test clinicians use to find out.

How the stretch test works

Stand in front of a mirror in bright, even light. Natural daylight near a window works better than overhead bathroom lighting, which exaggerates shadows. With clean hands, gently pull the skin of the lower eyelid downward and slightly outward, stretching it taut against the cheekbone. Watch what happens to the color.

If the darkness stays the same or looks about the same, pigment in the skin itself is the likely driver. Melanin does not move when you stretch skin, so brown or gray-brown discoloration that persists points to pigmented dark circles. This type is more common in people with medium to deep skin tones and often runs in families. Mechanistically, it involves excess melanin deposited in the epidermis, the dermis, or both, sometimes worsened by chronic rubbing, eczema, allergies, or sun exposure.

If the darkness gets worse or spreads when you stretch, the cause is more likely vascular. The skin of the lower eyelid is among the thinnest on the body, often under one millimeter. Stretching thins it further, making the network of small veins and capillaries underneath more visible. Vascular circles typically read as blue, purple, or reddish rather than brown, and they often intensify with fatigue, allergies, nasal congestion, or menstruation, all of which affect blood flow and vessel dilation in the area.

If the darkness largely disappears when you stretch, or when you tilt your head back and shine light directly on the area, you are probably looking at a structural shadow. This is not discoloration at all. It is a groove, usually the tear trough, where the lower eyelid meets the cheek. Volume loss with age, a naturally deep-set eye socket, or bulging fat pads above the groove create a valley that catches shadow in overhead light. No cream can lighten a shadow, because there is no pigment to lighten.

A second check: the photo test

To confirm a structural cause, take two photos of your face. One with light coming from directly above, one with light coming straight on, such as a flash at eye level. Structural circles look dramatic in the first photo and nearly vanish in the second. Pigmented and vascular circles look similar in both.

Why the distinction matters

The treatment logic follows directly from the mechanism. Pigmented circles respond, gradually, to ingredients that interrupt melanin production or speed its turnover: topical retinoids, vitamin C, azelaic acid, kojic acid, and diligent daily sunscreen, since ultraviolet exposure drives melanin synthesis. Chemical peels and certain pigment-targeting lasers are in-office options, though they require caution on deeper skin tones because aggressive treatment can trigger post-inflammatory hyperpigmentation, making the problem worse.

Vascular circles call for a different playbook. The goal is either to constrict or support the visible vessels or to thicken the overlying skin so they show through less. Caffeine-containing topicals produce a modest, temporary vasoconstriction. Retinoids used consistently over months can stimulate collagen and slightly thicken the eyelid skin. Managing allergies and nasal congestion matters more than most people expect, since congestion impairs venous drainage around the eyes. Cold compresses in the morning offer short-term improvement by narrowing vessels.

Structural circles are the type most resistant to skincare, because the problem is geometry, not color. Options range from makeup techniques that place a light-reflecting concealer inside the groove, to injectable hyaluronic acid filler placed by a qualified medical professional to fill the trough, to surgical repositioning of fat in more pronounced cases. Anyone considering injections in this area should know the tear trough is technically demanding and carries real risks, so credentials matter.

Mixed cases are the rule, not the exception

Many people stretch the skin and see the darkness partially fade but not disappear, or notice both a brownish tint and a bluish cast. Studies examining dark circle patients frequently find mixed presentations, which is one reason single-ingredient products disappoint. A mixed case may need a layered approach: sun protection and a pigment-directed ingredient for the melanin component, allergy control and skin thickening for the vascular component, and lighting-aware concealer or a professional consultation for the groove.

The stretch test is not a substitute for a dermatologist visit, especially if circles appeared suddenly, affect one side only, or come with swelling or pain. But for the common, chronic version most people live with, two minutes at a mirror can save months of buying products designed for a problem you do not have.
