Most under-eye treatments fail because people treat the wrong cause. Here is how clinicians sort dark circles into three categories, and how you can do a rough version of the same exam at home.

Walk into any dermatology clinic with dark circles and the first thing a good clinician does is not reach for a prescription pad. It is a physical exam, and it usually takes under three minutes. That is because the phrase dark circles describes at least three different conditions that happen to look similar in a bathroom mirror. Treating one as if it were another is the single most common reason people spend years on eye creams with nothing to show for it.

Dermatologists broadly sort periorbital darkening into pigmented, vascular, and structural types, with plenty of mixed cases. Each has a distinct mechanism, and each responds to a different set of interventions.

The pigmented type is caused by excess melanin deposited in the skin under the eye. This is more common in people with medium to deep skin tones, and it often runs in families. The mechanism can be genetic, but it can also be acquired: chronic rubbing from allergies or eczema triggers post-inflammatory hyperpigmentation, and unprotected sun exposure stimulates melanocytes in this thin, vulnerable skin. Pigmented circles typically look brown or brown-gray, and the color often extends onto the upper eyelid as well.

The vascular type is not about melanin at all. The skin under the eye is among the thinnest on the body, often under one millimeter. When the venous network beneath it is congested, dilated, or simply close to the surface, the skin acts like tracing paper over a bruise. These circles read as blue, purple, or reddish. They worsen with poor sleep, alcohol, salt, crying, and nasal congestion, all of which increase venous pooling or fluid retention. Allergies are a major driver here too, which is why some clinicians call the darkening under a congested child's eyes allergic shiners.

The structural type is not discoloration at all. It is a shadow. As the midface loses volume with age, or in people born with deep-set eyes and a pronounced tear trough, a groove forms between the lower eyelid and the cheek. Overhead lighting casts a shadow into that groove, and the eye reads it as darkness. No cream can lighten a shadow, which is why this type frustrates people the most.

The home exam, step by step. First, the stretch test. Gently pull the under-eye skin taut, downward and slightly outward, and watch the color. If the darkness fades or vanishes when stretched, the cause is likely structural or vascular, because you have flattened the shadow-casting groove or thinned the appearance of pooled blood. If the darkness stays exactly the same, pigment in the skin itself is the likely culprit, because melanin moves with the skin.

Second, the lighting test. Stand under a harsh overhead light, then face a window with soft frontal light. Structural circles change dramatically between the two, appearing deep under downlighting and nearly disappearing in frontal light. Pigmented circles look roughly the same in both.

Third, the color check in daylight. Brown or gray suggests pigment. Blue, purple, or red suggests vessels. A darkness with no real color of its own, just depth, suggests shadow.

Fourth, the history check. Did the circles worsen after a bad allergy season, a stretch of poor sleep, or a stuffy nose? Vascular. Have they been present since childhood and do relatives have them? Often pigmented or structural, depending on family pattern. Did they appear gradually in your late thirties or forties alongside a flatter cheek? Structural volume loss is a strong candidate.

Why the sorting matters. Pigmented circles respond to the same logic as other hyperpigmentation: daily sunscreen, treating the underlying rubbing or eczema, and clinician-guided use of ingredients such as azelaic acid, vitamin C, or low-strength retinoids, sometimes chemical peels or pigment-targeting lasers in office settings. Vascular circles respond to addressing congestion and fluid dynamics: managing allergies, sleeping with the head slightly elevated, moderating salt and alcohol, and in some cases procedures that thicken the skin or address prominent vessels. Caffeine-containing creams can transiently constrict superficial vessels, which is why they show modest, temporary effects on this type only. Structural circles respond to volume and contour approaches, most commonly hyaluronic acid filler placed by an experienced injector, or in some cases lower eyelid surgery. Makeup strategy differs too: pigment is neutralized with peach or orange correctors, while shadow is better addressed with light-reflecting products placed in the groove itself.

Most real-world cases are mixed, and the self-test is a screening tool, not a diagnosis. Sudden, one-sided, or rapidly changing darkening deserves a medical evaluation, as do circles accompanied by swelling, pain, or vision changes. But for the majority of people, three minutes with a mirror and a window will explain more about their dark circles than a shelf of products ever has, and it will make the next conversation with a dermatologist far more productive.
