The Stretch Test: How to Tell if Your Dark Circles Are Pigment, Blood Vessels, or Shadow
Most under-eye treatments fail for one reason: they target the wrong cause. A two-minute self-assessment used in dermatology clinics can tell you which of the three main types you actually have.

Dark circles are not one condition. Dermatologists who publish on periorbital hyperpigmentation generally sort them into three broad categories: pigmented, vascular, and structural. Each has a different mechanism, and each responds to a different set of treatments. A brightening cream built for excess melanin will do almost nothing for a hollow that casts a shadow, and filler placed into a hollow will not lighten skin that is genuinely darker. Before spending money on any of it, it helps to know which problem you are solving.
The good news is that the sorting process does not require special equipment. Clinicians rely on three simple maneuvers: stretching the skin, changing the lighting, and reviewing history. You can approximate all three at home in front of a mirror.
Step one: the stretch test. With clean hands, gently pull the under-eye skin downward and slightly outward, the way a clinician would when examining the area. Watch what happens to the color. If the darkness stays exactly the same, or looks worse, the pigment is likely sitting in the skin itself. That points to pigmented dark circles, driven by excess melanin in the epidermis or dermis. Common contributors include genetics, sun exposure, chronic rubbing from allergies or eczema, and post-inflammatory hyperpigmentation, which is more prevalent in medium to deeper skin tones. If instead the darkness fades noticeably when the skin is stretched, the color is probably not in the skin. It is coming from beneath it, which leads to the next test.
Step two: check for a vascular signature. The skin under the eyes is among the thinnest on the body, often under one millimeter. When it is thin enough, the orbicularis oculi muscle and the venous network beneath it show through as a bluish, purplish, or reddish cast. This is the vascular type. Clues include a color that shifts toward blue or purple rather than brown, worsening with fatigue, poor sleep, nasal congestion, or menstruation, and improvement when you press gently on the area for a few seconds and watch it blanch. Allergies matter here too: congestion slows venous drainage around the eyes, which is one reason so many people with hay fever report darker circles during pollen season.
Step three: the lighting test for shadow. Stand under a light source directly overhead, then look at the same area with light coming from straight ahead, such as a window at face level. If the darkness nearly disappears with frontal lighting, you are most likely looking at a structural circle, meaning a shadow cast by anatomy rather than a true color change. The usual culprit is a tear trough, the groove that runs from the inner corner of the eye toward the cheek. It can be present from a young age due to how the ligaments in that region attach, or it can deepen with age as midface fat volume declines and skin loses collagen. Under-eye bags make it worse, because a bulge above a groove exaggerates the shadow below it.
Why this changes what you should do. For pigmented circles, evidence-supported options work on melanin production and turnover: daily broad-spectrum sunscreen, topical retinoids, azelaic acid, vitamin C, and, under medical supervision, agents such as hydroquinone or certain chemical peels. Treating the trigger matters just as much, which often means managing allergies or eczema so the rubbing stops. For vascular circles, the levers are different: improving sleep, treating nasal congestion, using caffeine-containing topicals that temporarily constrict superficial vessels, and, in some cases, procedures that thicken the overlying skin so vessels show through less. For structural circles, no cream can fill a groove or remove a shadow. Options in that category include hyaluronic acid filler placed by an experienced injector, and for prominent fat pads, surgical evaluation.
One honest complication: most people are mixed. Studies of periorbital hyperpigmentation repeatedly find that a majority of patients have more than one type at once, for example a mild tear trough plus thin skin plus some sun-related pigment. If your stretch test and lighting test give partial answers in both directions, that is not a failed test. It is a finding, and it explains why a single product rarely delivers a dramatic result.
When to see a professional. If the darkness appeared suddenly, affects one eye only, is accompanied by swelling or pain, or does not fit any of the patterns above, a medical evaluation is warranted, since thyroid disease, anemia, and dermatologic conditions can all present around the eyes. For everyone else, the two-minute self-check is a reasonable starting point. Knowing whether you are fighting melanin, blood vessels, or a shadow will not fix the circles by itself, but it will stop you from buying answers to the wrong question.
