The Stretch Test: How Clinicians Tell Pigment, Blood Vessels, and Shadow Apart Before Treating Dark Circles
Most failed dark circle treatments start with a wrong diagnosis. A two-minute bedside exam, no equipment required, sorts the three main causes and predicts what will actually work.

Walk into any dermatology clinic with under-eye dark circles and a careful clinician will do something surprisingly low tech before recommending anything. They will gently stretch the skin below your eye, tilt your face toward a light source, and possibly press a glass slide against the area. This informal workup, often called the stretch test or pinch test, is the closest thing the field has to a triage tool. It matters because dark circles are not one condition. They are at least three, and each responds to a completely different intervention.
The three mechanisms behind the shadow. Dermatologic literature generally sorts periorbital dark circles into pigmented, vascular, and structural types, with many patients showing a mix. Pigmented circles come from excess melanin deposited in the epidermis or dermis, often driven by genetics, chronic rubbing, sun exposure, or post-inflammatory changes after eczema or allergies. Vascular circles come from the visibility of dilated or congested blood vessels beneath skin that is, in this region, among the thinnest on the body, often under one millimeter. Structural circles are not discoloration at all. They are shadows cast by anatomy: a deep tear trough, under-eye hollowing from fat loss, or a bulging fat pad above a groove. The skin itself may be perfectly normal in color.
How the stretch test works. Place a finger below the dark area and gently pull the skin downward and slightly outward, then observe in good frontal light. If the darkness moves with the skin and stays just as dark when stretched, melanin in the skin is the likely culprit. Pigment is embedded in the tissue, so displacing the tissue displaces the color. If the darkness becomes more pronounced when stretched, thinning the already thin skin further, the cause is probably vascular. Stretching reduces skin thickness and makes the venous plexus underneath even more visible. If the darkness improves or disappears with stretching, or vanishes when you tilt your chin up and face a light directly, you are most likely looking at a structural shadow. Changing the geometry removes the shadow because there was never any true discoloration.
Two confirming checks. Clinicians often add a second look with tangential lighting, holding a light source at a low angle across the face. Structural circles darken dramatically under angled light and soften under direct light, because shadows depend on light direction while pigment does not. The color itself is a third clue, though a softer one. Brown or tan tones lean pigmentary. Blue, purple, or reddish tones lean vascular, since deoxygenated blood viewed through thin skin scatters light toward blue, a phenomenon related to the same optics that make veins on the wrist look blue. A grayish darkness that changes with head position and time of day, often worse in the morning, suggests fluid congestion layered on a vascular base.
Why the distinction changes everything. Match the treatment to the wrong mechanism and results are predictably disappointing. Topical brighteners such as azelaic acid, vitamin C, kojic acid, or prescription hydroquinone target melanin production. They can meaningfully lighten a pigmented circle over eight to twelve weeks but will do essentially nothing for a shadow cast by a hollow. Vascular circles respond, when they respond at all, to strategies that thicken skin or reduce vessel visibility: retinoids to stimulate collagen over months, certain vascular lasers, or camouflage with peach-toned correctors that neutralize blue. Structural circles are a volume and contour problem. They typically require hyaluronic acid filler placed by an experienced injector, fat repositioning during lower blepharoplasty, or simply acceptance, because no cream changes bone structure or fat pad position. Mixed presentations, which are common, may need sequenced approaches.
Where self-assessment has limits. The stretch test is a screening tool, not a diagnosis. Several conditions mimic ordinary dark circles and deserve professional evaluation: periorbital dermatitis, chronic allergic rubbing that produces both pigment and skin thickening, thyroid-related changes around the eyes, and rarely, medication effects. Iron deficiency and poor sleep do not cause dark circles by themselves as often as folklore suggests, but they can worsen vascular prominence by affecting circulation and fluid retention. Anyone whose circles appeared suddenly, are asymmetric, or come with swelling or vision changes should skip the home tests and see a clinician.
The practical takeaway. Before spending on any product or procedure, spend two minutes at a mirror in daylight. Stretch, tilt, observe the color. If the darkness stays put when stretched and looks brown, pigment-directed topicals are a rational first step. If stretching makes it worse and the tone is bluish, focus on skin thickness and vessel visibility, and moderate your expectations for creams. If the darkness melts away when you change the light, you have a shadow, and no serum on earth treats a shadow. Getting the mechanism right first is the single highest-yield decision in the entire dark circle journey.
