The Stretch Test: How to Tell Which Type of Dark Circle You Actually Have
Most under-eye treatments fail because people treat the wrong problem. A simple at-home check, borrowed from dermatology exam rooms, can tell you whether your dark circles are pigment, blood vessels, or shadow.

Walk into any pharmacy and you will find a wall of eye creams promising to erase dark circles. What the packaging rarely says is that dark circles are not one condition. Dermatologists generally sort them into three mechanisms: pigmented, vascular, and structural. Each responds to different interventions, and a product built for one type often does nothing for another. Before spending money, it is worth spending sixty seconds figuring out which type you have.
The tool clinicians use for this triage is disarmingly simple. It is often called the stretch test or pinch test, and it works because the three mechanisms sit at different depths in the tissue. Pigment lives in the skin itself. Vascular discoloration comes from blood vessels showing through the skin. Structural darkness is not a color at all, it is a shadow cast by the shape of the under-eye area.
How to run the test. Stand in front of a mirror in bright, even light, ideally daylight from a window rather than overhead bathroom lighting, which exaggerates shadows. Gently place a fingertip below the dark area and pull the skin downward and slightly outward, stretching it taut against the cheekbone. Watch what the discoloration does.
If the darkness moves with the skin and stays just as dark, the pigment is in the skin. This points to a pigmented dark circle, sometimes called periorbital hyperpigmentation. The color is usually brown or tan rather than blue or purple, and it often appears on both the upper and lower lids. Common drivers include genetics, particularly in people with medium to deep skin tones, chronic rubbing from allergies or eczema, sun exposure, and post-inflammatory pigmentation after irritation. Because the problem is melanin, the logical interventions are pigment-directed: daily broad-spectrum sunscreen, treating the underlying allergy or itch so rubbing stops, and topical agents studied for hyperpigmentation such as vitamin C, azelaic acid, kojic acid, or retinoids used cautiously on thin eyelid skin. Vascular treatments and fillers will not touch this type.
If the darkness gets worse when you stretch, you are likely looking at a vascular dark circle. Stretching thins the already thin under-eye skin, making the network of veins and capillaries beneath it more visible. The color tends to be blue, purple, or reddish, and it often intensifies with fatigue, alcohol, salty food, menstruation, or nasal congestion, all of which affect blood flow or fluid retention around the orbit. A second confirming sign: gentle pressure with a fingertip briefly blanches the area, and the color refills within seconds. Management here targets the vessels and the skin covering them. Cold compresses constrict vessels temporarily. Caffeine-containing eye products can do the same for a few hours. Treating allergic rhinitis matters because congested nasal veins share drainage pathways with periorbital veins. Over the longer term, retinoids may thicken the dermis slightly, making vessels less visible, and some clinicians use lasers that target hemoglobin. Brightening creams aimed at melanin will disappoint this group.
If the darkness fades or disappears when you stretch, or when you tilt your chin up and shine light directly at the area, you probably have a structural circle. This is a shadow created by the tear trough, the groove where the lower eyelid meets the cheek. Volume loss with age, a naturally deep-set eye, or puffiness above the groove from fat pads or fluid can all deepen the shadow. No topical product can treat a shadow, because there is no discoloration in the skin to fade. The relevant conversations here involve volume and contour: hyaluronic acid filler placed by an experienced injector, or in some cases surgical repositioning of fat, along with managing morning fluid retention through sleep position and sodium intake.
The complication: most people are mixed. Studies of periorbital hyperpigmentation repeatedly find that a majority of patients have more than one mechanism at once, commonly pigment plus a structural trough, or vascular show plus mild pigment. If your stretch test gives an ambiguous answer, that is itself informative. Mixed cases usually need a layered plan, and this is where a board-certified dermatologist earns their fee, sometimes using a Wood lamp or dermoscopy to confirm where pigment sits.
One caution. Photograph yourself in consistent lighting before starting anything. Under-eye appearance fluctuates daily with sleep, hydration, and salt, and it is easy to credit a product for improvement that was really just a good night of rest. Give any topical eight to twelve weeks, the realistic timeline for pigment turnover or dermal change, before judging it.
The stretch test will not replace a clinical exam, but it does something valuable: it stops you from buying a brightening cream for a shadow, or a caffeine serum for melanin. Matching the treatment to the mechanism is the single most useful thing anyone with dark circles can do.
