The Stretch Test: How Clinicians Sort Dark Circles Into Three Types Before Treating Anything
Pigment, blood vessels, or shadow: most under-eye darkness traces back to one of three mechanisms, and a two-minute bedside exam usually reveals which one you have. Here is how the sorting works and why it changes everything about treatment.

Ask a dermatologist what causes dark circles and you will rarely get a single answer. That is because the term describes a visual result, not a diagnosis. In clinical literature, periorbital hyperpigmentation is generally sorted into three dominant mechanisms: excess melanin in the skin, visible blood vessels beneath thin skin, and structural shadowing from the anatomy of the lower lid. Most people have a blend, but one mechanism usually dominates, and identifying it is the single most useful step a patient can take before spending money on anything.
The two-minute exam. Clinicians typically start with a simple stretch test. Using a clean finger, gently pull the under-eye skin downward and slightly outward while looking in a mirror under bright, direct light. Watch what happens to the darkness. If the discoloration stretches with the skin and stays visible, the pigment likely lives in the skin itself, pointing to a melanin-driven type. If the darkness fades or shifts as the skin flattens, structure and shadow are probably doing most of the work. A second check involves lighting: look at your face under overhead light, then hold a light source directly in front of your face at eye level. Shadow-type circles soften dramatically with frontal light because the darkness is literally a shadow cast by a groove or bulge. Pigment and vascular circles barely change.
Type one: pigmented. This type appears brown or tan, often extends onto the upper lid or toward the temple, and is more common in people with medium to deep skin tones, particularly those of South Asian, Middle Eastern, and Mediterranean descent. The mechanism is straightforward: melanocytes in the periorbital skin produce more melanin, sometimes due to genetics, sometimes provoked by sun exposure, chronic rubbing from allergies or eczema, or post-inflammatory pigmentation after irritation. Because the problem is pigment in the skin, treatments target melanin production and turnover: daily broad-spectrum sunscreen, topical agents such as azelaic acid, kojic acid, vitamin C, or prescription retinoids, and in-office options like low-strength chemical peels. Fillers do nothing here, and lasers require caution in darker skin because aggressive energy can worsen pigmentation rather than clear it.
Type two: vascular. These circles read as blue, purple, or reddish, and often look worse in the morning, after poor sleep, or during allergy season. The lower eyelid has some of the thinnest skin on the body, often under half a millimeter, and in some people the orbicularis muscle and its vascular network sit close enough to the surface to show through. Fatigue and allergies worsen the look because venous congestion dilates those vessels. A quick clue: vascular circles often improve slightly with a cold compress, which constricts vessels, and worsen when you press and release, briefly showing a color shift. Management focuses on the skin barrier and vessel visibility: retinoids can thicken the dermis modestly over months, caffeine-based topicals produce a short-lived constriction effect, and treating underlying allergic rhinitis can reduce congestion. For persistent cases, vascular lasers or subtle filler placed as a camouflaging layer are options a qualified physician can evaluate.
Type three: structural or shadow. This is not discoloration at all. A groove called the tear trough, volume loss in the mid-cheek with age, or bulging under-eye fat pads creates a valley that catches shadow under most lighting. The stretch test and the frontal-light test both improve it. No cream can treat a shadow, which is why so many people report that expensive eye creams did nothing: they were treating geometry with topicals. Structural circles respond to interventions that change the contour, most commonly hyaluronic acid filler placed carefully in the trough, or lower blepharoplasty when fat herniation is the driver. Both carry real risks in this anatomically delicate area and belong exclusively in the hands of experienced injectors or oculoplastic and facial plastic surgeons.
Why the sorting matters financially. The eye-cream market is enormous, but most products are formulated for one mechanism at best. A brightening serum built around vitamin C targets melanin. It will not shrink a fat pad or thicken skin over dilated vessels. Matching the mechanism to the intervention is the difference between measurable improvement and a drawer full of half-used jars.
When to see a professional. If darkness appeared suddenly, affects one eye only, or comes with swelling, itching, or pain, see a physician, since thyroid disease, dermatitis, and other conditions can present around the eyes. Otherwise, run the stretch test and the light test at home first. Walking into a consultation already knowing whether you are fighting pigment, vessels, or shadow makes the conversation faster, the plan more targeted, and the outcome far more likely to satisfy.
